Birth Certificates:
To obtain a birth certificate in our Health Department you must have been born in Jackson County.
If you come into the office you will need to fill out an application and show us identification with your signature or
picture on it.
If you are not able to come to the office, you may apply for it by mail. You will need to send us the following:
Your full name at birth.
Your date of birth.
Your mother's full maiden name. (If adopted, give adoptive mother's full maiden name).
Your father's full name. (If adopted, give adoptive father's name.
If there has been a legal adoption, give us all the information that you know prior to the adoption.
The reason you need the birth certificate.
Sign your request with your legal signature, address and a daytime phone number.
A copy of your drivers license or identification with your signature on it.
A stamped, self-addressed envelope.
Enclose a money order payable to the Jackson County Health Department.
